New Trusted Assessment Facilitator roles being recruited

As part of Norfolk County Council’s promoting independence agenda they have introduced new roles that will support and influence discharge pathways. These roles will be based within the acute hospitals in Norfolk and will work closely with patients, families and providers along with health and social care staff.

The new service will focus on unblocking and streamlining processes to reduce delayed transfers of care and improve the experience of patients as they return to their home or move into a care home. It is also an opportunity to get providers involved in shaping the discharge pathways that are currently under development in the acute hospitals.

Potential candidates for the roles will already have a Level 3 Diploma/NVQ in Health and Social Care or equivalent, and understand local and national policies relating to the care sector. They will have experience of working in either a provider, hospital or a care home environment and will have been involved in the discharge process.

Closing date is the 3rd December 2017.
